Your Pain Treatment Appointment

We offer a team approach to patient care and pain relief. During your appointment, our doctors will meet you for your consultation. During this time, our board certified team of pain doctors will learn about the history of your pain and determine a course of action for your pain treatment. Our doctors wіll ѕtаrt bу аѕkіng questions аѕѕосіаtеd to уоur gеnеrаl hеаlth аnd thеn specifically to the nаturе and ѕуmрtоmѕ of thе knee pain, hip pain, neck pain, shoulder pain or back раіn. Once our team has identified the root cause of your discomfort, they will present you with a treatment plan to restore you back to health as soon as possible.

Pain Treatment FAQ

Cost and Insurance Coverage

Most pain treatments are covered by all major medical insurances and Medicare. Please call us to verify your insurance as we are in network with some insurances providers and out of network with others. Once you provide us with your insurance information, one of our team members will verify your coverage right away, and schedule an appointment at your earliest convenience. If you don’t have insurance, you are still welcomed to come in for an evaluation as a self pay patient, give us a call for details. 

Do I need a physician referral?

No. The majority of providers we work with don’t require a physician referral. However, some insurances do, please give us a call and our verifications team will help you get in touch with your insurance provider. 

What to bring to your initial consultation?

1. Insurance card or cards.

2. Photo ID.

3. Copies of any recent MRI, X-rays or EKG results. 

4. Complete medical history. List of all prescriptions and over-the counter medication that you are taking. 

5. List of allergies, including food and medical.
